Take a moment to write down the stories, memories, or anecdotes about your time in braces with Dr. Peterson and send them in to our office.

Contest is open to all current and former patients
Please include name, address and phone number on your essay
All entries must be received by 5:00 p.m. November 16, 2009
The winning essay will be chosen by Dr. Peterson and staff
The winner will receive a gift certificate to Trailhead Bike & Kayak in Ludington or The Outpost of Manistee (winner’s choice)

Instructions, Rules and Limitations

Watch the Ludington Daily News and Manistee News Advocate for six articles by Dr. Peterson.
You may pick up an official entry form and instruction sheet at either of our offices or print one from our website at www.familyorthosmiles.com.
Solve the clue given each week. When all five clues are put together you will have the pieces needed to solve an anagram.
Solve the anagram and submit to our office by 5:00 p.m. on November 9, 2009.
All entries must be submitted on an official entry form.
The official entry form must be complete and correct to qualify for the drawing.
A winner will be drawn from the correct entries.

Limitations:

Patient must have a good oral health clearance from his/her family dentist prior to start of treatment.
Orthodontic treatment is defined as Comprehensive, Limited or a Two Phase treatment plan designed by Dr. Peterson and would include diagnostic records prior to the beginning of treatment and one set of retainers after treatment is completed, plus retainer check appointments for up to 18 months.
No refund will be made if the patient transfers out of the practice during the course of treatment.
Contest is open to everyone age 7 or older at the initial examination. The winner may transfer the prize to an immediate family member only, in need of orthodontic treatment, i.e. brother, sister, mother, father, child.
One entry per person.
Treatment must begin within 12 months of the date of the drawing.
No cash value.
Dr. Peterson reserves the right to bill any available orthodontic insurance prior to and during treatment.

Is Back!

Bring in canned or other non-perishable food items during office hours through November 18th and receive 1 ticket for each item (limit 5 tickets per patient per day) for your chance to win a $50 gift card.
All food collected will benefit local food pantries.

Mondays & Tuesdays 8 a.m. to 5 p.m.
Thursday, October 16th 8 a.m. to 5 p.m.

Manistee Office Hours:
Thursdays 8 a.m. to 5 p.m.
Friday, October 17th 8 a.m. to 5 p.m.

We will be closed 10/9 to 10/15.

Our Second Annual Traveling Toothbrush Contest is now underway!

We'll be awarding significant prizes in the following categories: Toothbrush That Travels Farthest; Strangest Location/Most Difficult Location to Brush Your Teeth; and Funniest/Cleverest Toothbrush Photo.
